Platelet-Rich Plasma(PRP)

WHAT IS PRP?

PRP consists of two elements: plasma, or the liquid portion of blood, and platelets, a type of blood cell that plays an important role in healing throughout the body. Platelets are well-known for their clotting abilities, but they also contain growth factors that can trigger cell reproduction and stimulate tissue regeneration or healing in the treated area. Platelet-rich plasma is simply blood that contains more platelets than normal.

 

WHAT IS IT USED FOR?

PRP treatment can help support wound healing in trauma and joint injury. The technique can address male pattern baldness, stimulate the growth of hair transplants and enhance other cosmetic procedures.

HOW LONG PRP LAST?

The results of PRP treatment are most noticeable after several weeks for joint injections and six months for scalp injections and are not permanent. Thus  patients may require additional injections at the direction of their doctor.

SIDE EFFECTS?

Because the treatments use a patient’s own tissues, PRP injections are safe and can be administered alone or used in conjunction with other procedures.

 

WHAT IS A PRP INJECTION?

After creating platelet-rich plasma from a patient’s blood sample, that solution is injected into the target area, such as an injured knee or a tendon. In some cases, the clinician may use ultrasound to guide the injection. The idea is to increase the concentration of specific bioproteins or hormones, called growth factors, in a specific area to accelerate the healing process.

MRECHANISM OF HEALING

The healing mechanism behind PRP injections is not completely understood. Studies show that the increased concentration of growth factors in platelet-rich plasma may stimulate or speed up the healing process, shortening healing time for injuries, decreasing pain, and even encouraging hair growth.

WHAT PRP IS USED FOR?

PRP injections are used for a range of conditions,* from musculoskeletal pain and injuries to cosmetic procedures.

1.    Tendon, Ligament, Muscle and Joint Injuries:

 

PRP injections may be able to treat a range of musculoskeletal injuries and conditions. For example, chronic tendon injuries such as tennis elbow or jumper’s knee can often take a long time to heal, so adding PRP shots to a treatment regimen can help to stimulate the healing process, decrease pain and enable a return to activities sooner.

 

2.    Post-surgical Healing

Clinicians first used PRP to accelerate healing after jaw or plastic surgeries. Now, post-surgical PRP injections have expanded to help heal muscles, tendons, and ligaments, as procedures on these tissues have notoriously long recovery times.

 

3.   Osteoarthritis

Early studies indicate that PRP injections may help treat osteoarthritis pain and stiffness by modulating the joint environment and reducing inflammation, but research is growing.

 

4.   Hair Loss

PRP injections can be effective in treating male pattern baldness, both in preventing hair loss and promoting new hair growth. PRP can also aid in the stimulation of hair growth after hair transplants.

 

5.   Skin Rejuvenation

In conjunction with Microneedling and other procedures, PRP injections are sometimes used as an anti-aging treatment to reduce wrinkles and other signs of aging.

PRP RISK: 

PRP injection is a low-risk procedure and does not usually cause major side effects. The procedure involves a blood draw, so you should make sure you are hydrated and have eaten beforehand to prevent feeling lightheaded. After the procedure, you may experience some soreness and bruising at the injection site.

PRP SIDE EFFECTS:

Because PRP injections are made up of your own cells and plasma, the risk of an allergic reaction is much lower than with other injectable medications like corticosteroids. Less common risks of PRP injections include:

Bleeding, Tissue damage,  Infection and Nerve injuries

Dermal Fillers

WHAT ARE DERMAL FILLERS ?

Dermal fillers are gel-like substances injected under the skin to create a smoother or fuller appearance, or both. The FDA regulates dermal fillers as medical devices.

WHAT ARE THEY MADE OF:

Most dermal fillers are made of of hyaluronic acid,  a natural substance that the body eventually breaks down and absorbs. Thus, they are not permanent and may require additional injections to maintain the desired effect.

BOTOX vs. DERMAL FILLERS:

Botox and dermal fillers are non-invasive cosmetic procedures that can be used to reduce the appearance of wrinkles and combat the signs of aging. Dermal fillers tend to be less risky and last longer than Botox. Another advantage of dermal fillers is that some types are reversible. For instance,  hyaluronic acid-based dermal fillers can be safely dissolved  with hyaluronidase injections

HOW LONG DO THEY LAST?

The duration of effect can vary significantly depending on patient's age, the brand used, and the location of the injection site.  For example, dermal fillers might last for up to two years when used to add volume to the cheeks, but only six months when injected into the lips.

 

TYPES OF DERMAL FILLERS:

  .   Hyaluronic acid, a biodegradable sugar that is naturally found  in the body;

·         Calcium hydroxyapatite, a mineral and a major component of bone;

·         Poly-L-lactic acid (PLLA), a biodegradable, synthetic material;

·         Non-biodegradable polymethylmethacrylate (PMMA) beads suspended in a solution that contains bovine (cow) collagen. 

FDA-APPROVED USE OF DERMAL FILLERS:

     Dermal fillers are approved for specific uses in people aged 22 and older to:

·         Correct moderate-to-severe facial wrinkles and skin folds

·         Increase fullness of lips, cheeks, chin, under-eye hollows, jawline, and back of the hand

·         Correct acne scars.

FDA WARNINGS:

    .  The FDA has not approved injectable silicone or any injectable fillers for body contouring or enhancement;

​      . The FDA has warned against getting filler injected into the  breasts, buttocks, or spaces between the muscles because of serious injury -- pain, infection, permanent scarring or disfigurement, and even death.

​·         The FDA has not approved needle-free devices for the injection of dermal fillers and warns against using them to inject hyaluronic acid or other lip and facial fillers due to serious injuries and in some cases, permanent harm to the skin, lips or eyes have occurred.

​·         The FDA also warns against buying or using lip or facial fillers that are sold directly to the public due to potential contamination with chemicals and infectious organisms.

​·         FDA-approved dermal fillers are supplied by a prescription for injection by a licensed health care professional using a syringe with a needle or a cannula (a small flexible tubing with a blunt tip that is inserted under the skin).

POTENYTIAL SIDE EFFECT: 

usually subside in days following the injection 

·         Bruising

·         Redness

·         Swelling

·         Pain

·         Tenderness

·         Itching

·         Rash

LESS COMMON SIDE EFFECT:

 Inflammation such as swelling or redness may develop near the dermal filler injection site following viral or bacterial illnesses or infections, vaccinations, or dental procedures 

·         Raised bumps in or under the skin (nodules or granulomas) that may need to be treated with injections, oral antibiotics, or surgically removed

·         Infection

·         Open or draining wounds

·         A sore at the injection site

·         Allergic reaction

·         Necrosis (tissue death)

 SERIOUS SIDE EFFECT:

The most serious but low risk associated with dermal fillers is accidental injection into a blood vessel to potentially cause skin necrosis (death of tissue), stroke, or blindness.  This underlines the need for an experienced medical provider.
